At Patient Centered Health Care, we have effective and competent nursing staff who serves individuals ages 13 and up. We provide them with the first level of medicine, including but are not limited to the following:
- Physicals
- Cancer Screening
- Immunization/Flu Shots
- Preventive Medical Procedures for General Illnesses and Injuries
- Sexual transmitted disease treatment
We help them manage their disease and prevent the worsening of their current condition. With client and family coordination, we can effectively determine the care they need to improve their condition.
Our main role is to carry out initial assessment and prevention procedures to address the illness and/or injury of the client before referring them to a hospital, such as measuring their height, weight, and blood pressure, and other basic recordings and examinations. We also provide direct patient care after the initial diagnosis.
